Expanding Wellness Services

The medically-integrated Wellness Center is a community-based health and wellness complex centrally-located in the Live Oak district of Santa Cruz. This welcoming, accessible space is open to all and serves disabled and chronically-ill patients in our communities.

Navigating the health care system is inherently challenging for all. Care coordination is vital for ensuring senior patient safety because it keeps multiple providers in the loop regarding medications and can reduce duplicative tests and treatments.

Many suffer from chronic diseases like heart disease, hypertension, and diabetes, and are at high risk for complications.

Also, the Personal Enrichment Program (PEP) is available at the new Wellness Center, offering in person and virtual-live activities and classes to support physical, social, emotional, and cognitive well-being. In addition to fostering improved health, personal growth, and creativity, the programs are designed to keep individuals engaged and active in the community.
